Care Coordinator
Inspire Lives Care Services Ltd Leicester
Inspire Lives Care Services is seeking a dedicated Care Coordinator to join our team. The Care Coordinator plays a vital role in ensuring the provision of high-quality care services to our clients.
Duties- Conduct initial and ongoing assessments of patients' healthcare needs, preferences, and goals
- Develop individualized care plans based on assessment findings, in collaboration with patients, families, and healthcare providers.
- Monitor and adjust care plans as needed to meet changing patient needs.
- Facilitate communication between patients, families, and healthcare providers to ensure that all parties are informed and involved in the care process.
- Prepare and submit reports on patient outcomes, care activities, and service utilisation as required by the organisation.
- Ensure compliance with organisational policies, procedures, and regulations related to patient care.
- Work closely with multidisciplinary healthcare teams to ensure coordinated and holistic patient care.
- Previous experience in an office setting is essential
- Strong communication and interpersonal skills
- Excellent organisational abilities
- Ability to work well under pressure
- Empathy and compassion for clients' needs
- Providing care: 1 year (preferred)
- Driving Licence (preferred)
