Job Purpose:

We are seeking a dedicated and experienced Community Nurse to join our Client's Integrated Transfer of Care (ITOC) team. This role involves contributing to holistic assessments for adults with complex physical and medical needs, including those at risk of falls, and with neurological, musculoskeletal, and frail conditions.

The primary goal is to collaborate with key stakeholders and community services to prevent hospital admissions and support timely discharges, ensuring effective discharge planning and oversight to avoid re-admissions.

Key Responsibilities:

  • Assessment and Treatment: Provide high standards of assessment, treatment, and advice to patients, carers, and families.
  • Clinical and Discharge Risk Management: Skillfully manage ongoing clinical and discharge risks.
  • Specialist Discharge Planning: Lead in the assessment, diagnosis, and formulation of specialist discharge plans for patients with complex needs, including Advanced Care Planning and End of Life care.
  • Autonomous Practice: Perform all aspects of clinical work independently.
  • Specialist Advice: Offer specialist advice within multi-disciplinary bed base facilities and patients’ homes.
  • Referrals: Refer patients to other health professionals as necessary.
  • Supervision and Teaching: Supervise and teach less experienced nurses, Assistant Practitioners, students from various disciplines, and Social Services carers.
  • Service Development: Contribute to service development and strategic planning using specialist knowledge.
  • Audit and Research: Lead or participate in audits and research, disseminating findings and recommending practice changes.
  • Collaboration: Ensure close collaboration with carers and health and social care agencies.

Qualifications and Experience:

  • Proven experience in a similar role, preferably within community services.
  • Strong skills in managing complex discharge planning and clinical risks.
  • Ability to work autonomously and within a multidisciplinary team.
  • Excellent communication and leadership skills.
