Bury Community Occupational therapy are looking to recruit a Band 5 Occupational Therapist to assist in the assessment and treatment of patients in the community who have a wide variety of health needs.
The post holder will participate in the delivery of specialist assessment and rehabilitation programmes to community based patients across a range of acute and long term conditions including Palliative care and Rheumatology.
They will work as part of an interdisciplinary team within the community therapy service.
An ability to travel to different geographical locations across the Trust footprint in line with service needs is required. A full UK driving licence and access their own vehicle is therefore essential for this community role.
  • To carry out appropriate assessment and treatment of allocated patients.
  • To gain consent, undertake assessment, make clinical judgement, and formulate and implement treatment plans with support from Senior Occupational Therapists, when required.
  • To formulate, implement and evaluate treatment plans in response to assessment and need.
  • To work in close liaison with other disciplines involved in the care and treatment of patients allocated.
  • To provide clinical support to Community OT Services across specialities. Working flexibly to meet service requirements. This may include Enhanced Intermediate Care / Palliative Care / Rheumatology under the guidance of senior clinicians.
