We’re looking for someone enthusiastic and compassionate to join our friendly and supportive team, caring for older adults in the Maidenhead community. This is a full-time role (37.5 hours per week) based at Nicholson House, Maidenhead, with regular home visits to clients – so you’ll need access to a car.

About the role:

You’ll play a key part in delivering mental health care and treatment to older adults, including those living with dementia. You’ll work across our Community Mental Health Team (CMHT), Home Treatment Team (HTT), and Memory Clinic, managing your own caseload and working closely with colleagues from health, social care, and voluntary sectors.

Main duties of the job
  • Work collaboratively as an integral part of a dynamic multi-disciplinary team, managing a diverse caseload of patients with care and compassion.
  • Conduct thorough assessments of patients, ensuring active involvement of their caregivers and family members in developing comprehensive care plans.
  • Take ownership of formulating, implementing, and assessing the effectiveness of care plans for a specific group of patients, including those with complex needs, in close collaboration with patients and the multi-disciplinary team.
  • Offer direct supervision and guidance to Support Workers and/or students, fostering their growth and development within the team.
  • Provide expert advice to team members and external agencies regarding the unique needs of service users, contributing to the overall improvement of service delivery.
