We require enthusiastic and motived individuals to work as part of the Single Transfer of Care Team - Central Hub. The role of a Hub Coordinator (office based) is fast-paced, busy, with changing priorities, and we are seeking staff to join this highly valued team.

For all posts there will be a requirement to actively support the administrative processes for referral management, coordination and support.

If you have a passion for supporting patients, enjoy the challenge of problem-solving and using your initiative on a daily basis, whilst liaising with many different levels of staff; and patients’/carers, then we would like to hear from you.

The right candidate will have a natural interest in being a committed member of a busy team, providing support with the planning and facilitation of admissions and discharges for all patients, and be confident and able to work with minimum supervision at times of pressure.

We pride ourselves on high service delivery and patient-centred care. A willingness to be flexible across the service we deliver is required, and you will need to be both proactive and reactive to keep the flow of patients moving through the system.
