Main duties of the job
The successful applicant/s will be responsible for managing referrals into our services and providing evidence-based care to support patients in their own homes. To assess, plan, implement and evaluate individualised patient care promoting self care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity is vital.
Providing advice, guidance and support to junior staff in the delivery of high quality care and actively supporting the District Nurse/Senior Community Nurse in the management of the health care environment are integral to this role..
This great opportunity includes working alongside members of the MDT including Therapy Teams, General Practice and Specialist Services .
