Service Lead - Older Adults Pathways

We are developing our Older Adult offer within the Shropshire, Telford and Wrekin and are looking for someone to lead on the mobilisation of this.
This is an exciting development opportunity for someone who has experience in a management/leadership role and is looking for an opportunity to expand their experience in strategic and operational leadership.
This specialist post will require you to have a good understanding of the business of the service and be committed to delivering at a senior management level.
You will need to have a good understanding of older adult needs and the work taking place around neighbourhood health approaches for this population.
Please note - this will be a fixed term post for 9 months initially.

Main duties of the job- Leading on the delivery of the mobilisation of the care groups older adult development and expansion programme
- Operational leadership across the Community Older Adult Services, supporting the day-to-day operation of the services, ensuring that ongoing delivery is coordinated and consistent.
- Developing and maintaining key relationships with system partners, as well as wider community and inpatient teams and their lead across the care group to support the development of multi-disciplinary and agency models to support seamless pathways and care closer to home

Detailed job description and main responsibilities
The post-holder will work with the Senior Leadership team to plan and oversee the mobilisation and implementation of Older Adult Services and Pathway Developments ensuring that all service provision is coordinated, evidence based and effective.
The post holder will lead on the ongoing delivery providing strategic and operational oversight for these services.
The post-holder will promote the service and manage positive multi-agency relations particularly with key stakeholders.
The post-holder will ensure the service is able to meet compliance with clinical and operational standards and contractual requirements and ensure regular reporting links are in place with the ICB and required Trust Networks.
The post holder is responsible for overall service standards (quality and safety and service user satisfaction and meaningful involvement) and effective joint clinical governance in conjunction with the Quality and Governance Lead.
The post-holder is responsible for implementing relevant service strategies which promote engagement and access for the seldom heard and harder to reach communities, including minority ethnic, cultural and faith communities and other sub-populations where there are health inequalities and impoverished health outcomes.
Lead on service wide projects / developments as agreed with the Head of Operations.
The post holder will ensure that there is active service user involvement in the design, delivery and evaluation of the service.

Qualifications
Essential criteria- Professional Qualification in Mental Health, Nursing, Social Work/Occupational Therapy - and/or significant experience of operational leadership
- Leadership/Management Qualification or evidence of demonstrable leadership and Management experience
- Leadership Development Programme

Experience
Essential criteria- Proven experience of managing complex services with a track record of successful delivery
- Extensive experience of working in mental health or older adult services including experience of working closely with wider system partners, service user groups and other community providers, including primary care
- Extensive experience of managing a related service and delivering a high-quality service which delivers on outcomes
- Proven track record of leading an implementation process including completing complex, multi-faceted implementation plans
- Experience of all aspects of workforce management and recruitment
Skills
Essential criteria- Involvement in project implementation
- Ability to influence and negotiate good outcomes with senior managers from partner agencies, commissioning agencies and Trust corporate services, where there may be resistance to change.
Knowledge

- Detailed knowledge of clinical interventions and best practice in clinical services
