We are seeking a Community Nursing Team Lead to work within the South Bedfordshire Locality.

Detailed job description and main responsibilities

There are 2 posts available, 1 full time 37.5 hours per week and 1 part time 21 hours per week.

Person specification

Education

Essential criteria
  • Registered General Nurse
  • Educated to degree level or equivalent
  • Post-registration qualification in clinical field eg: SPQDN (or working towards SPQDN)
  • Nurse Prescriber
  • Relevant teaching/mentoring qualification and evidence of teaching skills
  • Demonstrable evidence of significant post-registration nursing experience in a community setting.

Experience

Essential criteria
  • Evidence of and commitment to multi-professional working
  • Demonstrate working within a multi-agency environment
  • Have a comprehensive understanding of partnership working
  • Demonstrate application of clinical governance
  • Excellent understanding of community health and social care principles and practice
  • Understanding of National and Local issues that impact on the local population. Comprehensive understanding of the roles of others in health and social care.
  • Experience in managing multi-disciplinary staff.
  • Experience in managing issues related to conflict resolution avoidance.
  • Knowledge of the principles of Safeguarding of Vulnerable Adults and Children.
  • Knowledge of the principles of Health Promotion and prevention of illness
  • Experience of using electronic systems to manage activity and the delivery of KPI information.
  • Evidence of specialist holistic assessment and evaluation skills
  • Ability to prioritize and manage own time to meet the needs of the service
  • Ability to collect and interpret data in relation to patient activity and nursing care
  • Demonstrate experience in monitoring of staff and quality of service provided.

Knowledge and skills

  • Demonstrate evidence-based practice and up-to-date clinical skills
  • Experience of clinical leadership
  • Extensive experience of complex assessment of health and social needs
  • Ability to oversee multiple caseloads and monitor quality
  • Demonstrate excellent communication skills using a variety techniques and strategies, i.e. email, documentation, active listening, verbal.
  • Ability to disseminate complex sensitive information to others

Our Community

We provide a wide range of community health and mental health inpatient services to children, young people, adults of working age, and older adults in The City of London, Hackney, Newham, Tower Hamlets, Bedfordshire and Luton. We also provide psychological therapy services to the London Borough of Richmond, and provide Forensic services to the whole of North East London.

We provide services in urban and rural settings, sometimes in our inpatient units, but mostly in the community close to where people live. Integrated care is key in making the best use of resources and providing effective care.
