Reablement Support Worker - Community Based

You will be working as a vital Reablement Support Worker within the Intermediate Care Service within Bury Council. This position is 100% community focussed within Bury and very local, surrounding areas. These positions are supporting adults who are in a stage of medical and/or mental health reablement within their own homes.

These are all adults who have recently been discharged from a hospital or a bed based reablement unit, that may not have the family or friend network, in the short term to fully support them back to independence in their own homes.

It is essential to succeed in this position to be a driver, with access to a car and be comfortable working with people in their own homes.

These are FT positions working in a split shift pattern that mirrors the needs of the individuals we support. The shift pattern is Week 1, Monday, Tuesday, Saturday, Sunday and Week 2: Wednesday Thursday Friday HOURS are 8.30am-1pm and 4.30-9.30pm.

Our support workers:

  • Provide person centred, therapeutic and creative solutions to aid recovery and encourage independence.
  • Deliver, evaluate and adjust reablement programmes to meet goals and required recovery timelines.
  • Work as part of a multi-disciplinary team of services across health, social care and other community partners.
  • Maintain a safe and stimulating environment for people in reablement.

Requirements

We are looking for support worker that has experience in providing home care that are ideally qualified to level 2, or willing to develop skills and abilities. You will have experience with moving and positioning adults and are trained and efficient in the distribution of medications.

You will work in a small team that will manage their own case load. We need you to be self-motivated and hardworking. This is a very busy service that will require you to be both physically and mentally well. To really aid reablement we need you to be energetic, enthusiastic and have a natural ability to encourage and influence positive behaviour changes.

You will need to communicate well with others and be proficient in the use of technology to support care plans and reporting.

This position reports into a Community Reablement Manager/Team Lead and you will be employed by Bury Council on a Band 7, £25,584 per annum £13.26 per hour. You are entitled to a generous council paid holiday entitlement and benefits allowance which includes;
