The Community Independence Service in Westminster is looking for a motivated and enthusiastic Band 6 Physiotherapist to join our friendly Community Rehabilitation Team. You will be passionate about the value of community and rehabilitation, have had clinical experience and be ready to step into a community role to further develop your clinical and non-clinical skills.

The community setting is THE PLACE TO BE with care being prioritised at home and patients being cared for in the community wherever possible - this is clearly outlined in the NHS Long Term Plan and can already be seen on the ground.

We are part of a wider Community Independence Service, which is also offered in Hammersmith & Fulham and Kensington & Chelsea. You will be connected with peers across the tri-borough to support you and to work on service improvement partnerships.

You will also be working closely with our other CIS teams, and acute partners, such as Imperial College Healthcare NHS Trust and Chelsea & Westminster Hospital NHS Foundation Trust.

Main duties of the job

The Community Independence Service (CIS) is a provider partnership which focuses on hospital admission avoidance; where clinically appropriate care can be provided in the community, facilitation of early supported discharge from hospital, maximising independence and reducing dependency on longer term services.

The band 6 post holder will see clients newly discharged from hospital, use evidence based/patient centred principles to assess, plan and deliver individual treatment plans. The post holder will undertake all aspects of clinical duties as an autonomous practitioner, with supervision from senior therapists within CIS.

The post holder will be expected to promote, maintain and develop a high quality, efficient and effective physiotherapy service to clients in accordance with the philosophy and operational policies of the team. The post holder will be encouraged to develop skills in providing undergraduate placements to students.

Detailed job description and main responsibilities
  1. JOB SUMMARY

The Community Independence Service (CIS) is a provider partnership which focuses on hospital admission avoidance where clinically appropriate care can be provided in the community, facilitation of early supported discharge from hospital, maximising independence and reducing dependency on longer term services.

You will deliver skilled physiotherapy care to clients in their own home, based on a thorough holistic assessment of their clinical and personal needs and the agreement of an individualised rehabilitation care plan.
  1. MAIN DUTIES AND RESPONSIBILITIES
  2. 1 Clinical
  • To be professionally and legally accountable for all work undertaken. To work within the standards and guidelines of the team, the Trust, the CSP Code of Professional Values and Behaviour and the HCPC Standards of Conduct, Performance and Ethics.
  • To undertake physiotherapy assessment of clients, (suitable to band & experience) with diverse needs through subjective questioning and objective assessment.
  • To prioritise factors involved in the clients presenting condition through clinical reasoning based on knowledge of current professional opinion, clients prognosis and other physical, social emotional and mental health issues.
  • To set goals for treatment with clients and/or their carer and ensure that care is client centred and agreed by the client and/or their carer.
  • To set and maintain a high standard of clinical practice which is evidence based, using standardised assessment and outcome measures where appropriate
  • To independently screen and assess multi-disciplinary referrals from a variety of sources. This screening requires individual responsible action when at times limited other professionals are currently involved. This screening will establish priority, determine needs and initiate a plan of intervention.
  • To manage a caseload of clients with a range of needs, utilising supervision and guidance as necessary from senior staff. To successfully monitor, evaluate and review physiotherapy treatment/interventions within a multi-disciplinary framework and to modify as necessary taking a key worker role as indicated.
  • To use knowledge of other multi-disciplinary roles and external agencies to ensure effective referral management and achieve best client care.
  • To have the ability, knowledge and skills required for safely moving and handling people in community environments.
  • To ensure client and service records are recorded and maintained on IT databases and that client documentation is according to trust and local policies.
  • To prescribe, delegate and monitor the work of rehabilitation assistants and counter sign documentation.
  • To educate and teach relevant carers/health workers to participate in on-going rehabilitation care programmes for clients, as appropriate.
  • To actively be involved in service development, with support and guidance from senior therapy staff.
  • To manage clinical risk in own caseload and ensure that rehabilitation assistants and students comply e.g. manual handling risk assessment and local policy regarding lone working.
  • To maintain and update a Continuing Professional Development Portfolio (CPD).
  • To evaluate own practice through audit and outcome measures with support of senior therapists when required
  • To utilise opportunities to develop skills and knowledge, apply critical appraisal and keep up to date with current practice in the field and attend accredited courses.
  • To participate in the in-service training, post registration training, lectures and education programmes to other therapists, other disciplines and to visitors to the service.
  • To act as professional practice educator for physiotherapy students.
  • To participate in education programmes for client groups.
  • To actively participate in health promotion and health education initiatives
  • To comply with all mandatory training: fire, conflict resolution, CPR, manual handling, child protection and safeguarding of vulnerable adults.
  1. 1 MANAGEMENT
  • To ensure that accurate and evaluative records of patient care and statistics are maintained in accordance with the service policy.
  • To participate in the individual performance review system.
  • To provide supervision, annual appraisal and monitoring the work of junior staff including Band 5s, rehabilitation assistants and students with support from senior staff.
  • To provide informal day to day support to junior staff members.
  • To be responsible for the safe use of all equipment.
  • To ensure that all equipment defects, accidents and complaints are reported via the Datix System and escalated to senior staff for further resolution if required.
  • To comply with Trust policies and procedures.
  • To participate in the collection of statistics and audit programmes as required by the service.
  • To participate in the induction of new staff members.
  • To attend, actively participate in and as required initiate multi-disciplinary meetings and case conferences.
  • To attend physiotherapy staff meeting and other relevant meetings.
  • To use time management skills, to respond to demands from clients and staff on daily basis.

Person specification

Education and Qualifications

Essential criteria
  • Degree/ Diploma Physiotherapy
  • Registration with HCPC as a Physiotherapist
Desirable criteria
  • Member of CSP
  • Post registration courses s e.g. Neurology, Older people/elderly rehab , Goal setting , Student Placement Educator
  • Membership and/or involvement in a CSP professional network
  • Post graduate MSc degree

Previous Experience

Essential criteria
  • Previous experience of working in the UK or a similar health economy as a physiotherapist.
  • Previous general experience in older people/elderly rehab, orthopaedics, general surgery, falls, dementia, respiratory
  • Experience of teaching juniors/rehab assistants
  • Experience of completion of audit and service developments
Desirable criteria
  • Experience of working as an autonomous practitioner in a community setting
  • Previous general experience to include the following: neurology, community working, pain management, group work
  • Experience of teaching students
