INTRODUCTION The post is an appointment for 6 months initially with a possible further extension as a Junior Clinical Fellow in Trauma & Orthopaedics at Alder Hey Childrens NHS Foundation Trust. 1 THE DEPARTMENT The Alder Hey Trauma and Orthopaedic Department is one of the largest quaternary referral centres in Europe for paediatric orthopaedic patients.
Fracture Clinics are held daily from Monday to Friday inclusive, taking referrals from the Emergency Department, GPs, and other hospitals. Elective general paediatric orthopaedic clinics are also held daily. There are a number of specialist clinics that run in the G5 location alongside the regular elective orthopaedic clinics but under direct consultant supervision.
These include a baby hip clinic that delivers a selective ultrasound hip screening programme for residents of Liverpool, normal variant clinic for toddlers, adolescent knee disorder clinic, adolescent disorder hip clinic, cerebral palsy clinic, and Ponseti clinic for clubbed feet.
Multi-disciplinary clinics are also held at regular intervals with paediatric neurologists, paediatric rheumatologists, plastic surgeons, geneticists and metabolic physicians. The orthopaedic ward resides on Ward 4A. This is a 32-bed unit for Orthopaedics and Neurosurgery with two 4-bed bays for intensive monitoring of post-op patients after spine and major hip/ lower limb surgery.
4A admits Orthopaedic and Neurosurgical patients almost exclusively. We have dedicated elective orthopaedic theatres with both day-case and inpatient capacity in addition to paediatric orthopaedic trauma theatres which run daily. The North West Movement Analysis Centre is a unique strength of the department.
Facilities exist for 3-D instrumented gait analysis, EMG, Force plates and dynamic Pedobarography. The centre also co-ordinates pre- and post-operative management of patients. Alder Hey benefits from a full-time in-house experienced orthotist who can fabricate/ provide the entire range of braces and splints for the extremities and spine.
There are physiotherapists working throughout the hospital on wards and in clinics. There are extensive links between Alder Hey physiotherapy and the community/ school physiotherapy teams to facilitate management of children with neuro-disability.
The Orthopaedic Department benefits from close links with the Advanced Nurse Practitioners and physiotherapists who act as Extended Role Practitioners. ERPs in this department run Baby Hip clinics, Normal Variant Clinics, Adolescent Sports/ Knee disorders Clinic and Cerebral Palsy follow-up clinic.
There are separate liaison teams for the CP, Limb Reconstruction and Neuromuscular patients. Working closely with the Consultants, they co-ordinate pre-operative preparation and post-operative rehabilitation. The Paediatric Surgery department also works closely with Advanced Nurse Practitioners across surgery and a Specialist Nurse Urology team.
The department also has two Physician Associates who are in developing roles and run their own clinics working closely with Consultants. 3 DUTIES Full time (48 hours/week) Junior Clinical Fellow - equivalent to F2, CST1, CST2 level (Core Surgical Trainee) within the division of Surgical Care at Alder Hey Childrens Hospital.
The roles available is as follows: Orthopaedic Surgery (with out of hours Neurosurgery cover) Our aim is to support you to progress in a career within surgery and gain clinical experience in paediatric trauma and orthopaedic surgery. The post holder will be allocated an educational supervisor who will act as an advisor and mentor.
The posts offered are available for 6 months with the possibility of further extension to 12 months. The posts involve on-call commitments with out of hours supplements payable in accordance with the Trusts Terms and Conditions of Employment for Locally Employed Doctors.
The Research and Development Directorate ensures that research is incorporated into the management infrastructure and is part of the Trusts overall service plan and strategy. The Trusts Research and Developmental Manager, along with research group leaders, promote research within the overall strategic aims.
A number of operational groups and key individuals facilitate the planning coordination and support of research. Areas of direct relevance to child health in which there exists a critical mass of expertise are focused around research subject groups.
The Research and Development Manager, research group leaders and a nurse lecturer practitioner also provide support to new initiatives which fall outside the strategic focus, especially those with potential to develop into programmes of research.
Quality assurance of all Trust based research is achieved through peer review of all proposals by the Research Committee. The Trust runs a number of evidence-based medicine research teaching sessions and Research Awareness Day Workshops which are open to all Trust staff disciplines.
Research participation is encouraged, and the post holder can be given a project to carry out with a view to publication within the timeframe of the post. 5 OTHER INFORMATION Library facilities There is 24 hour access to library facilities, which take 140 current periodicals, both medical and nursing.
Literature search and inter-library loan services are available as well as access to Medline, Cochrane and CINAHL databases. An integrated hospital information system provides access to information sources and the internet. Applicants should have completed their Foundation Year 1 (or equivalent). The posts are based in a brand new state of the art hospital with modern, unparalleled facilities for working and education. The appointees would gain a wide experience of working in surgical paediatrics in a nationally recognised childrens hospital, providing opportunities for training and education with a view to developing a future application for national selection in a career in any surgical or paediatric discipline.
