Practice Nurse BASE: Any SPCT site RESPONSIBLE TO: Lead Practice Nurse ACCCOUNTABLE TO: Clinical lead JOB SUMMARY: To contribute to the successful delivery of the key outcomes of the SPCT business plan by provision of a high quality nursing service to a defined practice population.
This will include assessment, health promotion, chronic disease management, health screening and treatment in line with the professional competence of the post holder. To be involved with the practice delivery of National Service Framework targets and local Health Improvement priorities.
