We have a Clinical Support Worker Higher Level, Therapy Assistant post, to work within the Redcar and Cleveland PCN Community Therapy Team to provide rehabilitation to patients in the local community.

This is a unique partnership between Health and Social care in South Tees with a commitment to working together, improving the quality of our services and getting rid of existing barriers between health and social services.

Support and in-service training will be provided to develop your skills, knowledge and understanding of the role. There are supervision structures in place to provide support and ensure that you continue to deliver the best possible service to the patients in our care.

Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.

Main duties of the job

Working in the community, you will work alongside Physiotherapists and Occupational Therapists to implement rehabilitation programmes in patients’ own homes, care homes and group settings, working closely with statutory and voluntary services to promote and maintain independence.

As well as completing joint visits with therapists the post holder will be expected to undertake independent visits to implement exercise programmes, complete mobility practice, self-care practice, provide, fit and check equipment as requested by the therapist.

At present the service is provided 5 days a week (Monday to Friday) with no requirement to work weekends or bank holidays. The post does require successful candidates to be able to travel independently within the local community to visit patients at home.

South Tees Hospitals NHS Foundation Trust is one of the largest teaching hospital trusts in the country, close to the North Yorkshire Moors and within easy reach of Newcastle and Leeds. It comprises two acute hospitals, James Cook University Hospital and Friarage Hospital Northallerton and a number of community hospitals and community services.

Therapy services at South Tees NHS trust are also part of the wider Integrated OT service.

Qualifications & Training

Essential criteria
  • 3 A-levels OR Health and Science T-level OR equivalent level 3 qualification(s) and Functional Skills (level 2) OR As a level 3 apprenticeship, such as the Senior Healthcare Support Worker apprenticeship (or other level 3 qualification pathway) AND/OR Short courses and completion of competency workbook equivalent to Level 3 in a range of patient and clinical care duties AND/OR Equivalent knowledge and experience in a range of clinical care duties equivalent to Level 3;
  • Theoretical knowledge
  • Experience of a range of care and related procedures
  • clinical observations
  • knowledge of relevant legislation

Experience

Essential criteria
  • Experience of working in health or social care
  • Experience of undertaking and recording clinical observations and escalation of changes or findings.
Desirable criteria
  • Experience of working as a therapy assistant.

  • Ability to deal with a wide range of people at all levels, including clients, carers, family, health and social care colleagues and other professionals.

Knowledge & Skills

  • Time-keeping, planning and prioritisation skills.
  • Basic IT skills to use electronic patient records and other Trust systems.
  • Analytical and judgement skills to make decisions based on patient / service user interactions and observations and metrics.
  • Understanding of the importance of robust, accurate, clear, timely documentation.
Desirable criteria
  • Working knowledge of Occupational Therapy or Physiotherapy
