About the Role:Worcestershire County Council is seeking a passionate and dedicated Personal Advisor to join their Care Leavers Service. In this key role, you will support young people aged 17–24 to transition successfully to adulthood. You will work closely with young people, placing them at the heart of decision-making, and help them achieve independence and a bright future.
You will be the allocated worker and key contact for your young people, offering practical, personal, and emotional support. This is a dynamic role where no two days are the same, and you will have the opportunity to build meaningful relationships while contributing to positive outcomes.
Key Responsibilities:
Develop and maintain respectful, supportive relationships with young peopleCreate and implement Pathway Plans in collaboration with young people
Keep accurate case records and maintain up-to-date documentation
Complete regular visits to young people at home and in the community
Attend meetings and work closely with partner agencies to ensure the best possible outcomes for your young people

Skilled in building effective working relationships and collaborating with others
Able to travel across Worcestershire and beyond must have own vehicle
