To provide professional and clinical expertise in the specialty of Tissue Viability and act as a resource for education and clinical expertise for Dorset County Hospital NHS Foundation Trust.

Can deputise in the absence of the lead Tissue Viability Specialist Nurse for the provision of services to patients at risk from and with Pressure Ulcers throughout the Trust which includes risk assessment from admission to discharge.

Able to assess all types of wounds including; trauma, leg ulcers, Diabetic and surgical and Pressure Ulcers and be able to review plans of care.
To provide up to date specialist evidence-based care, knowledge, advice and education for patients with Tissue Viability needs.

To work independently at a level appropriate to clinical competence and within the scope of professional practice.

The post holder will support the Lead Tissue Viability Specialist Nurse with the monitoring of pressure ulcer data via Datix and audit.

Document individual patient assessments, advising on future care and treatment.

Communicate advice to multidisciplinary team, patients and carers on a level that everyone can understand.

Assist with the planning, delivery and evaluating of an education programme in relation to Tissue Viability.

Maintain high standards of professional practice and competence in keeping with Trust policies, guidelines and NMC Code of Conduct.

Assist the Lead Tissue Viability Nurse in the development, implementation and monitoring of guidelines, protocols and standards relating to Tissue Viability.

Maintain a professional portfolio and take responsibility for own educational needs.

The post holder is required to have physical skills obtained through practice, in wound management and dressings, including Negative Pressure Wound Therapy application and application of Compression therapy.

This post is required to provide effective evidence-based care for patients referred to the Tissue Viability Service under the supervision of the Lead Tissue Viability Specialist Nurse. By using personal effectiveness and knowledge contribute to the development and delivery of high quality and cost-effective services.

To focus on the prevention of Pressure Ulcers within Dorset County Hospital in line with ongoing quality metrics.

Be an integral part of the ongoing Quality Improvement work occurring within the Tissue Viability Service.

To provide Education to Clinical staff in the appropriate management of Pressure Ulcer management and prevention.

Assist the Tissue Viability Service with caseload management; processing referrals and escalate concerns to the Lead Tissue Viability Nurse Specialist.

Be responsible for supply and stock management for the Tissue Viability Team.

To assist with implementation of Quality Improvement projects associated with Tissue Viability.

Provide clear and concise verbal communication to patients, clients and multidisciplinary teams.

Ensure all documentation applicable for job role is recorded accurately, legibly signed and dated in accordance with trust policies and procedures

Communicates effectively with a range of people including the MDT.

Listens to and respects the needs of patients and/or colleagues

Works as part of the MDT to ensure holistic care for patients and their relatives

Role models the Trust's behaviour standard addressing/reporting the behaviour that is not acceptable.

To communicate with the Patient Safety Team to ensure appropriate investigations are being carried out.

Able to professionally challenge staff to ensure good quality care is upheld.

Have a good understanding of the Mental Capacity Act and when best interest decisions are made. As well as accepting that courageous conversations are undertaken with Patients regarding their compliance with prescribed care.

Be able to provide Patient's with a greater depth knowledge in order to educate and empower them in regards to their ongoing care.

Be able to carry out a thorough and in-depth Patient assessment to include assessment of Patient's Skin, wounds, Equipment provision, Continence and Nutrition, as well as helping to plan Patients' discharges.

To able to effectively respond to changes in daily workload.
