Our aim is to ensure that people experiencing a mental health crisis are offered assessment and intervention quickly and effectively within the least restrictive environment as an alternative to hospital admission.

Main duties of the job

The Home Treatment Team (HTT) provides a a 7 day a week service, between the hours of 8am and 10pm, alongside a twilight shift which is 5.30pm until 3am for gatekeeping.

Our key responsibilities are around the assessing and delivery of care for people who are in crisis with their mental health. The team is multi-disciplinary and has a strong ethos of team working and team support, in order to deliver high quality care.

The post holder will work on a shift basis over a 7 day period. The HTT works closely within the crisis network in ensuring that good liaison and joint working with other crisis provision teams are maximized.

The band 6 social worker will work within the Home Treatment Team. The post is to provide clinical care to Service Users who are in Crisis. The HTT is multidisciplinary and will involve working alongside other clinical professionals as well as the Crisis Network and other Community Mental Health Teams.

As a band 6 the role with involve supervision of junior staff and participating in team development.

You will be involved in the assessment of Service Users in Crisis. leading on crisis assessments and decision making with onward referring to appropriate teams utilizing skills as a social worker to navigate pathways for optimal care and support.

Planning interventions, delivering a variety of Acute Crisis interventions, whilst reviewing care and planning for discharge.

Detailed job description and main responsibilities

Shift patterns are-

Days - 08.00 - 18.00 hrs with 30 min break

Late - 12.00 to 22.00 hrs with 30 minutes break

Job purpose

The post holder will work within a multi-disciplinary service under the supervision of experienced staff, and core responsibilities will include:

  • Comprehensive assessment of new referrals in crisis
  • Contributing to the crisis system on a rota where duties include assessment, intervention planning and onward care plans for people in crisis
  • Rotate onto shift coordinator where duties include taking and receiving of referrals, prioritising work load for the team, receiving and responding to telephone calls. Overall running of the shift under the support of the Senior Practitioners.
  • Carrying out initial HTT visit to set up plan of care and interventions for the team
  • Link to other clinical areas such as inpatient and CMHT’s • Delivering supervision to more junior members of staff
  • Contribute to service development

The post holder will be required to work in accordance with the HTT working hours which are currently 4 shifts of 8am-6pm, 7 days a week.

General requirements
  • Maintain Social work England registration / re-validation and adhere to professional guidelines and code of conduct.
  • Work within the framework of relevant policy and legislation both local and national.

Principal duties

Clinical
  • Triage and determine urgency of new referrals to the service
  • Carry out comprehensive assessments of an individual’s health and social care needs in a variety of settings and agree the most appropriate course of action
  • To assess, evaluate and manage clinical risk
  • To formulate a plan of care based on identified need
  • Deliver evidence based crisis interventions as appropriate
  • Review effectiveness of care plan at regular intervals
  • To work collaboratively with service users, carers and partner agencies
  • To coordinate the clinic work of a day by day basis
  • To maintain computer held care records in an accurate and timely fashion • Undertake all work in a culturally sensitive manner .
  • Develop effective working relationships with inpatient, community and voluntary organisations.
  • Develop and maintain effective working relationships with Primary Care colleagues.
  • Taking the lead in managing individuals with complex including mental health restrictions such as 37/41 and CTO and Social Circumstance Reports where appropriate and in line with named worker relationship.
  • Co-ordinate meetings, case conferences act as chair where required
  • Maintain accurate and contemporaneous clinical records in line with data quality guidance, legislation and policy.
  • Provide profession-based support and advice across the MDT and within multidisciplinary meetings
  • Lead on MCA, Best Interests and take the supporting lead in any identified appropriate DOLS.
  • Be engaged in proceedings related to the MHA
  • Work collaboratively with the MDT to lead work around CTO’s
  • Take an active part in 117 Aftercare support and planning.
  • Support social care assessments where complexities around health related matters are identified
  • Signpost and support housing and PIP applications where information is required
  • Provide support/liaison under Section 7 to provide information in CIN meetings via verbal and written support and/or advocate a patients needs where holistic mental health information is required
  • Health Interventions including but not exhaustive WRAP Planning, Psychosocial Interventions, Graded Exposure, Psychologically Informed Approaches (DBT, CBT, SCM, Trauma Informed, Stabilisation
  • Medication Management – Report back issues raised by service user with regards to potential side effects from medication, Seek support where required (i.e. where medication changes are necessary)
  • Liaison with other services MARAC, MAPPA,
  • Support and lead guardianship proceeding where appropriately identified.
  • Ability to navigate the social care pathway to understand the offer from local authority and how this links with health
Supervision
  • Regularly review own clinical work with supervisor to promote efficacy and contribute to the overall management of the HTT
  • Deliver supervision and facilitate annual PDR with more junior members of staff
  • Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution
  • Participate and contribute to specific social work supervision with Lead Social Worker
  • Participate in offered continuous professional development relating to Social Work.

Service development

Education and Training
  • Act as mentor to learners, keep up to date with relevant course and curriculum developments, and maintain ‘sign off’ status
  • Participate in training relevant to role, including training needs for the team.
  • Promote insight and understanding of mental health issues to service users, carers, and other agencies
  • Contribute to audit, research and evaluation as appropriate
Service Development

Qualifications

Essential criteria
  • Evidence of continued professional development
  • Registered Social Worker in Mental Health
  • Experience of working as a Band 5 or 6 Mental Health Social Worker in a community and MDT setting

Knowledge & Skills

Essential criteria
  • Working knowledge and understanding of Care Programme Approach & Care Management
  • Knowledge and understanding of appropriate therapeutic approaches
  • Knowledge and understanding of PDR and supervision processes as a supervisee
  • Demonstrable risk assessment and risk management skills
  • Demonstrable caseload and time management skills
  • Ability to work autonomously as well as a co-operative and effective member of the multi-disciplinary team with an ability to communicate at all levels
