Join our team as a Care Co-ordinator/Senior Support Worker and take the lead in planning, coordinating, and monitoring person-centred support across our Floating Support service. What you’ll do: Coordinate care plans and ensure timely, person-centred support delivery.Supervise and guide support staff to maintain high standards of care.Liaise with families, advocates, and health professionals to meet individual needs.Manage rotas, monitor service quality, and ensure compliance with CQC and Care Act standards.Support individuals to access community resources, build life skills, and achieve personal goals.

What we’re looking for: Experience in care coordination or supervising support teams.Strong organisational and communication skills.Knowledge of safeguarding, risk assessment, and person-centred planning.IT skills including database use to manage rotas and service user information PLEASE NOTE THIS ROLE DOES NOT OFFER SPONSORSHIP
