Practice Nurse

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels: Chronic Disease Management which should include Diabetes, (plus Asthma, COPD, AF, CKD but not essential) Doppler testing and stocking fitting Develop, implement, and embed health promotion and well-being programmes Care of chronic wounds, pressure and leg ulcers Routine acute wound care Travel advice and immunisation following patient groups directions Undertake cytology, womens health screening Family planning contraception ECGs both acute and routine as required Phlebotomy, as and when needed Hypertension Management Routine adult and child immunisations following patient group directions Chaperoning or assisting another clinician as requested Assisting clinicians with minor procedures if required Prepare and send samples for lab testing Removal of Sutures/Staples Urinalysis for both acute and chronic conditions Action cytology results as appropriate In addition to this you would be expected to on day-to-day basis/ as needed: Maintain accurate clinical records in conjunction with extant legislation Prioritise health issues and intervene appropriately Support the team in dealing with clinical emergencies Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice) Support the clinical team with all safeguarding matters, in accordance with local and national policies Communicate effectively with patients and carers recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating Recognise and work within own competence and professional code of conduct as regulated by the NMC Deliver opportunistic health promotion where appropriate
