Mental Health Support Worker
LOCATION: Tolvaddon,Cornwall
SALARY FROM: £14.45/hr

The Role:
- Providing support to customers living in the community who have severe and enduring mental health diagnoses
- Working closely with specialist providers to deliver support to customers, helping them to further independence, access local services, and enjoy being part of their local community
- Providing appropriate care in line with each customers' personal Support plan
- Being a Key Person for supporting people with health needs, including addiction
- Developing and maintaining relationships and being comfortable liaising with referral partners (local Mental Health Teams and Adult Social Care in Cornwall Council)
- Continuously assessing needs and risks appropriately to ensure the service remains suitable for the customers
- Ensuring caseload management systems are kept up to date
- Completing and regularly reviewing Risk Assessments
The Candidate should have:
- An understanding of support needs and support planning.
- Experience of risk assessing and risk management.
- A great communicator with the ability to develop relationships
- Accurate record keeping and computer skills
- Confident in lone working.
- Able to work under pressure and meet deadlines.
- Knowledge of welfare and housing benefits.
- A full understanding of safeguarding.
SUMMARY:
This role is being offered on a full time, 2 Month contract with possibility of extension and will be field based in the Cornwall area.
This role will be offered with an essential car user allowance, as a car is essential and driving between customers is a large part of the role
