Responsible to: Head of Service Delivery

Key Relationships: General Practice, One Westminster, Central and North West London NHS Foundation Trust (CNWL), Central London Community Healthcare NHS Trust (CLCH).

BACKGROUND

Healthcare Central London (HCL) is the GP Federation covering the Central London CCG area. The organisation supports 31 General Practices and 4 Primary Care Networks (PCNs). We operate a number of NHS contracts including a Care Navigation Service, a Clinical Pharmacy Service, a Community Dermatology Service and Partnership in Practice (PiP) - an out-of-hospitals provision which is sub-contracted to its 31 General Practice members.

JOB ROLE

We are looking to recruit HCAs to join our friendly team located at South Westminster Centre to deliver a face-to-face out-of-hours care for our registered patients in the Enhanced Access service.

Enhanced Access operates 18:30 - 20:00 weekday evenings and Saturdays 09:00 - 16:00.

You would work within a multi-disciplined team including GPs, ANPs, HCAs and Nurses supported by an Admin team of Care Coordinators.

We would welcome applicants that are experienced, friendly, flexible and can work as part of a multi-disciplinary team.

KEY TASKS AND RESPONSIBILITIES
  • Undertake, record and follow guidelines for the tasks for which you have received appropriate training and achieved competency
  • Undertake and record:

o Blood pressure and pulse

o Height and weight body mass index

o Urine testing

o Measurement of blood glucose

o Performing ECGs

o Peak flow measurement

o Spirometry

o Smoking status

o Health checks

o Weight management advice
  • Take and prepare pathology samples which will include, venepuncture, INR and urinalysis.
  • Wound Management: meet the needs of the patients presenting for wound care following the dressing formulary and care plan initiated and reviewed by the practice nurse.
  • Administer B12, Influenza, Pneumonia and shingles injections under the provision of Patient Specific Directives.
  • Assist in the provision of minor surgery and surgical procedures
  • Chronic disease management assist with the monitoring and management of patients under the direction of a suitably qualified healthcare professional.
  • Give accurate and appropriate information to patients
  • Assist the patient and other clinicians in the delivery of care in urgent or emergency situations, including initiation of effective emergency care
  • Act as chaperone and assist patients
  • Perform other clinical tasks as trained and agreed to meet the needs of the Practice as required within your general role as Health Care Assistant
  • Prepare and maintain environments and equipment before, during and after patient care interventions according to Infection Control guidelines
  • Infection control monitoring: assist with audits as required.
  • Maintain and control stock levels; ensure that adequate stock levels in all clinical rooms are maintained and rotated. Ensure daily, weekly and monthly checks are completed.
  • Physically assist in the transfer and movement of materials and equipment
  • Assist with health promotion campaigns
  • Maintain accurate and complete records and other documentation in accordance with Practice protocol
  • Be aware safeguarding, recognise when patients are at risk and raise concerns appropriately
  • Assist with the training of other staff members and with the education of doctors and nurses in training as appropriate
  • Work with the practice population and wider community to promote healthy living and encourage uptake of services including assisting in raising awareness and promotion of health

PERSON SPECIFICATION

Skills and Knowledge
  • Strong interpersonal and communication skills
  • Experience with phlebotomy, ECGs, wound care, measuring physiological observations (BP, pulse, peak flow, BMI etc), IM injections under PSD, urinalysis, health checks and spirometry.
  • Experience delivering health promotion
  • Stock maintenance
Desired experience
  • Experience working in a GP practice or patient facing environment.
  • Excellent SystmOne skills. HCL uses SystmOne.

Healthcare Assistants will be contactable by phone and email during the core opening hours for South Westminster Centre, 08:00 - 20:00 weekdays and 09:00 - 16:00 Saturdays.
