Croydon Health Services is seeking qualified Health Visitors (and student Health Visitors) who are keen to develop their leadership skills and to shape the future of Public Health Nursing in Croydon.

We are seeking practitioners who are very keen to work and learn as a team, who are warm and engaging with a sound understanding of the Healthy Child Programme and Safeguarding policy and practice. We have an exciting development within our service through our vibrant learning and development team.

We work with 2 Universities to support the development of Band 5 Nurses to complete Public Health modules, which will support those wishing to complete the Specialist Community Public Health Nursing qualification.

Context: This role is part of a skill mix team delivering the Healthy Child Programme and other additional support to children 0- 5 years old and their families. Embedding an integrated approach towards Health Visiting for families across Croydon Health Services.

Please note: An additional recruitment and retention premia of £2,000, paid in two instalments during the first year and the service is open to flexible working through discussions with line managers and ensuring service needs are met.

The post holder will lead and deliver the Healthy Child Programme by:
 •  Working to and managing a designated and defined case load of families, the Health Visitor will be the named health professional responsible for the planning and delivery of the HCP, reporting to and being line managed by a Team Leader or Clinical Service Lead (CSL)
 •  Planning their own workload effectively to meet client needs, clinical activity targets and KPIs
 •  Working in partnership with the team members, infant feeding colleagues, service users, Croydon Health Service (CHS) staff, and all stake holders, to promote the health and wellbeing of Children, Young People (age 0-19 years) and their families
 •  Working in partnership with the CSL, Practice Educator and multidisciplinary teams in the development of innovative clinical practice in line with the Healthy Child Programme
 •  Assess client needs, using the Family Strengths and Needs, Early Help Assessments and Risk Assessment tools (as appropriate) to plan, implement and evaluate evidenced based care given and support families where there is a chronic illness or disability and to advise on care in cooperation with other agencies
 •  Liaise effectively with colleagues within Croydon Health Services and partner organisations to ensure care for children and families is timely, well-coordinated.

Croydon Health Services NHS Trust provides hospital and community services from a number of community and specialist clinics throughout Croydon.

These include:
 •  Croydon University Hospital
 •  Purley Hospital
 •  The Sickle Cell and Thalassaemia Centre in Thornton Heath
 •  Community bases at Purley, Sanderstead, New Addington, Broad Green, Woodside
 •  A minor injuries unit in New Addington
