Description

As a Short Breaks Officer, you will play a crucial role in providing support and respite to individuals with disabilities and their families. In this dynamic and rewarding position, you will be responsible for coordinating and facilitating short breaks activities, ensuring that each individual’s needs are met and their quality of life is enhanced.
You will work closely with families, service users, and a multidisciplinary team to develop and implement personalized plans that promote social integration, independence, and well-being.

With a strong commitment to person-centered care, you will be passionate about creating opportunities for individuals with disabilities to engage in meaningful activities and build social connections. You will have a deep understanding of the unique challenges they may face and actively seek to empower them to achieve their goals.

Responsibilities

Develop and implement person-centered care plans for individuals with disabilities, ensuring their needs and preferences are considered at all times.
Coordinate and organize short breaks activities, including outings, social events, and recreational programs.
Support service users during short breaks, providing assistance and supervision as required.
Liaise with families and caregivers to ensure a smooth transition and continuity of care for service users.
Collaborate with external stakeholders, such as community organizations and healthcare professionals, to enhance the short breaks provision.
Conduct regular assessments and evaluations of service users' progress and adjust care plans accordingly.
Provide emotional support and advocacy for individuals with disabilities and their families.

Requirements
Bachelor's degree in Social Work, Psychology, or a related field.
Proven experience working with individuals with disabilities, preferably in a similar role.
Sound knowledge of person-centered care principles and implementation.
Ability to develop and maintain strong working relationships with service users, families, and professionals within the field.
Excellent communication and interpersonal skills.
Strong organizational skills and ability to manage multiple tasks concurrently.
Compassionate, patient, and empathetic nature.

Hours Per Week: 36.00

Start Time: 09:00
End Time: 17:30
Pay Per Day: £15.76
