Community Nurse - Ipswich West | East Suffolk and North Essex NHS Foundation Trust

Colchester Hospital University NHS Foundation Trust | Ipswich | findajob.dwp.gov.uk |
ESNEFT are paving the way with their huge advances within Integrated Community Services (INT’s). Come and join one of the Integrated Neighbourhood Teams within Ipswich and East and play your part in keeping patients safe and well within the comforts of their own homes.

An exciting opportunity has arisen within the Integrated Neighbourhood Teams covering Ipswich West . There are two posts one in IP1 and one in IP2.

We are looking to recruit a Community Nurse on a permanent basis for a full time position, part-time hours would also be considered.

These are diverse caseloads which are lead by a band 7 clinical lead and three full-time band 6 senior nurses.

We are fortunate to be co-located with our other community nursing, therapy and social care colleagues as well as our out of hours nursing and therapy colleagues. We also enjoy the benefits of having the post graduate education team and legacy nurse to support in achieving the community specific clinical skills.

The service is delivered between 8am and 6pm 7 days a week.

Many of the 8 INT teams are co-located with our social care colleagues and wider MDT providing huge benefits to patient outcomes. An improved INT structure has allowed for huge advances in not just delivering frontline care but a concentration on making longer term health and social care plans through local data and the development of local delivery plans.

The INT’s cover both beautiful rural geographical areas as well as the more urban population of the local towns providing holistic and sometimes complex nursing care within people’s homes.

You will not only be working alongside other Community and District Nurses but also with a larger team of skilled Generic Support Workers, Physiotherapist, Occupational Therapists, social workers, independence wellbeing practitioners, social prescriber, Matron, care co-ordinators and team administration colleagues.

You will be committed to team work, delivering high quality patient care and have a desire for on-going professional development. This is an opportunity to work within knowledgeable experienced teams and wider MDT.

You should be flexible, motivated and able to work within a changing environment. We will provide support through in-house professional forums, Mandatory training, 1:1 and personal development meetings as well as clinical rolling skills training via the practice development facilitators.
We facilitate students on training and have close working relationships with neighbouring community teams in Health and Social Care

We are ESNEFT and we provide hospital and community health services to almost one million people across east Suffolk and north Essex. Our dedicated staff deliver care from acute hospitals in Colchester and Ipswich, community hospitals, surgeries, community clinics and in patients’ own homes.

We are one of the largest NHS organisation in England, employing more than 12,000 staff.

We pride ourselves on supporting our staff. We offer a wide range of training and development opportunities, as well as flexible working options.

Along with supporting you to achieve your career goals we offer a generous pension scheme, unsocial hours payments (where applicable), 27 days annual leave on commencement (pro rata) and access to a range of NHS discounts. Our Staff Health and Wellbeing programme offer a variety of services.

Our philosophy is thatTime Mattersto everyone. Across the Trust, we concentrate on improving the things we do and removing those which cause time delays for our staff and patients.

We are investing in our commitment to Time Matters with a partnership with leading electronic patient record (EPR) supplier Epic. This digital transformation will bring what’s widely regarded as the world’s best EPR system to ESNEFT, transforming life in hospital for staff and patients.

If you are passionate about patient care and want to develop your skills and knowledge, then we want to hear from you.
