We require a skilled & experienced Band 6 Older Adult Community Mental Health Nurse to work within our RITT Team based at Balladen House, Rawtenstall, overlooking the beautiful Rossendale Valley.

The Rapid Intervention and Treatment Team supports patients predominantly aged over 65 years who may be experiencing a Functional or Dementia type illness. The team offer short term interventions and crisis support. The role also operates a Care Home Liaison function offering support in relation to behaviour management strategies for behavioural and psychological symptoms of dementia.
The role involves working alongside the Initial Response Services regards Older Adult Referrals.

The team is multi-disciplinary led, and works closely with other mental health services and teams in providing a holistic healthcare model.

Potential candidates ideally should have the relevant Older Adult /Adult Mental Health community experience, and have proven clinical skills matching with that of a Community mental health service.

We are looking for a candidate who has excellent knowledge and experience, and is able to demonstrate a positive attitude for change and to the care of the Older Adults- sat within the community setting.

The ability to manage risk, liaise with & communicate effectively is essential for the role, and applicants should be able to demonstrate sound working knowledge around the current national and local drivers which underpin community transformation programmes.

Candidates will be able to demonstrate resilience and the ability to manage the demands of a busy community team. Be able to offer management and clinical supervision to band 5, band 4, band 3 staff, and act as a practice assessor to support student activity.

There is a robust supervision and personal development structure in place via the service manager and the successful candidate will have development opportunities in house and externally.

Knowledge and skills around the coordination role is desirable in line with the MDT approaches and processes, models and initiatives, which are underpinned by clear crisis & care plans, incorporating risk assessment and management plans.

To act as a key member of the multi-disciplinary team ensuring the effective assessment, planning, monitoring and evaluation of care given to service users with severe and enduring mental illness under the care programme approach process whilst liaising with a wide range of multi-agency professionals.

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
