An exciting opportunity has arisen for a suitably qualified and experienced person to be part of the Brook Place team working on the Pathways Project, working with homeless service users across Liverpool City Centre.

The Pathways Project works alongside people who have/are rough sleeping and experiencing multiple disadvantages and social exclusion. The approach of the project is intensive, flexible, and person-focused support, with the aim of supporting people onto a sustainable treatment pathway and to resolve their homelessness.

The successful applicant will need to be able to work independently but engage with the wider pathways team managing complex and challenging cases, and have patient care at the forefront of their working practice.

Pathways recovery workers deliver harm reduction interventions, and carry a small caseload to deliver quality, tailored structured treatment on an outreach/in reach basis. This will include planning inpatient detox and residential rehabilitation journeys.

You will be working in psychologically informed framework which will include training and development, clinical supervision, and reflective practice.

The essential qualities and skills for this work are compassion, inclusivity, and resilience, being able to build authentic trusting relationships, and effective joint-working in a complex health and social care system.

A minimum of 2-years experience working within substance misuse services is essential.

Working out of the main site at Brook Place you will be part of the Liverpool Pathways Team working with difficult and often challenging service users.

 •  The post-holder will be an integral member of a multi disciplinary team delivering comprehensive clinical & psycho-social intervention services. The post-holder will carry a case load and provide screening, comprehensive assessments, care planning & reviews and a range of recognised psycho-social interventions and support through an individuals recovery journey.
 •  The focus of all interventions will be:
 •  To reduce substance related harm to the individual and wider community
 •  To promote healthier life styles and increase service user choice
 •  To promote and support individuals into both abstinence based and medically assisted forms of recovery
 •  To provide effective case management and coordination of the individual’s treatment plan
 •  To promote carer and service user involvement In fulfilling these aims the post holder will use recognised evidenced based interventions and up to date validated information and will have specific responsibility for the development, delivery and coordination of a recovery focused model.
 •  The post-holder will ensure that service users consistently receive high quality care and treatment that adheres to best practice guidelines and achieve high professional standards.
