PCN Clinical Pharmacist

Working with the whole medicines management and pharmacy team to ensure that patients receive a high level of care and service delivery. To work as part of the primary care team in the practice supporting the staff to help achieve efficiencies and effectiveness.
To support with leading improvements to maximise safe, cost effective best practice in prescribing to improve the quality of patient care within the practice. To improve repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines.
1.To work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas. 2.Take responsibility for the care management of patients with chronic diseases and undertake clinical structured medication reviews to proactively manage people with complex polypharmacy.
3.To ensure patients have appropriate monitoring tests in place when required. 4.Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients and to help in tackling inequalities.
5.Provide leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services.
6.Support patients to take their medications to get the best from them, reduce waste and promote self-care, through structured medication reviews. 7.Support the integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
8.Assist and lead where appropriate in identifying cost effective and evidence-based use of medicines. 9.Answer medicine related enquiries from GPs, practice staff and patients. Suggesting and recommending appropriate solutions and provide follow-up for patients to monitor the effect of any changes as indicated.
10.Participate in prescribing audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the practice team. 11.Analyse, interpret, and present medicines data to highlight issues and risks to support decision making across the primary care team 12.Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets) 13.To assist in reducing medication errors such as Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

14.Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved. 15.To abide by the General Pharmaceutical Societys code of ethics, observing and applying all professional, ethical, and legal obligations 16.To contribute to the PCN delivery of the highest possible quality standards, the NHS Long Term Plan, Quality and Outcomes Framework, Investment and Impact Fund and locally commissioned enhanced services 17.To maintain computerised records in accordance with data protection principles to ensure patient confidentiality.

18.To keep up to date with current pharmacy practice, new drugs, and their uses 19.To attend the practice meeting when required 20.To attend the Prescribing Leads Meetings, lead by the ICB. 21.Develop relationships and work closely with other pharmacy professionals across PCN and the wider health and social care system.
