Care Coordinator
JOB PURPOSE: To oversee/maintain the care of the Domiciliary Service’s Clients and take responsibility for the day-to-day running of the Domiciliary Service.
CORE RESPONSIBILITIES
• Ensure all staff members contribute to the best of their ability to the efficient running of the Domiciliary Service.
• Cover client care calls as per weekly/daily rota on Care Planner.
• Conduct spot checks, supervisions, client reviews & feedback, medication, moving & handling competency checks.
• Facilitate the practical induction of all new care & support workers.
• Ensure to carry out monthly support calls to assigned service users and update the system.
• Assess social and health care needs of new clients and maintain ongoing assessment and review of all other clients (in the absence of, or as requested by, the Registered Manager).
• Liaise with senior staff members and care staff to ensure that the highest standard of service is provided to service users.
• Allocate suitable alternative care & support workers when an existing care & support worker notifies that they are unable to attend a call.
• Undertake direct domiciliary care as required. Assist clients in all aspects of their care needs.
• Practice maximum integrity in all dealings with clients' personal and financial affairs and avoid abuse of the privileged relationship that exists with clients.
• Undertake initial risk assessments within clients’ homes in line with the Company’s Health & Safety policies and procedures as requested by the Registered Manager.
Requirements
• MUST have a driving licence
• Must have care experience (2 years)
