Liaison Professional for Complex Needs

The post holder will work under the supervision of the Lead Professional for Complex Needs and co-ordinate their activities to ensure that an efficient service is provided. Support the development of practice with service leads and others as appropriate.
Act as a specialist in practice development issues to improve care for patients with a Complex Need. Work closely with all members of the Complex Needs Team to ensure the teams objectives, that are aligned to the complex needs forward plan and complex needs delivery group action plan, are met.
Provide advice and support on the mental health act to ward staff and support the receipt and scrutiny of Mental Health Act Paperwork. Maintain excellent communication networks and relationships within and externally to the Trust. Raise the profile of the health care needs of people with a complex need across secondary care provision.
Bridge the gap between community and secondary services to enable better communication and access to healthcare for people with complex needs. Assess and advise accordingly on the issues surrounding the needs of some people with complex needs, on their admission to hospital care settings.
Facilitate the meeting of individual healthcare needs for people with complex needs. Work in partnership with people with complex needs, self-advocacy groups, and carers groups and Wakefield learning disability partnership board in the development, implementation and maintenance of service provision and change.
