Essential Awareness of accountability of own and others roles in a general practice surgery Knowledge of health promotion strategies Awareness of local and national health policy Knowledge of clinical governance issues in primary care Knowledge of infection prevention and control Good interpersonal and communication skills Passion for high quality patient safety and care Clinical skills wound management, venepuncture, ECG Communication skills, both written and verbal Management of patients with long- term conditions including Asthma, COPD and Diabetes Knowledge of population health issues Knowledge of public health issues in the area Clinical skills cervical cytology, immunisation and vaccination, minor illness, contraception, travel health Annual long- term conditions reviews, including Asthma, COPD and Diabetes Ability to support patients to change lifestyle Stock management Experience Experience in nurse-led management of long-term conditions Recent primary care nursing experience Qualifications Registered Nurse with relevant nursing/health degree Post graduate education in the management of diabetes, asthma and COPD Skills Strong knowledge of human anatomy and physiology Proficient in providing patient care and performing clinical procedures Excellent communication skills to effectively interact with patients and healthcare team members Ability to prioritize tasks and manage time efficiently in a fast-paced environment Attention to detail to ensure accurate documentation and medication administration Please note that this job description is not exhaustive and additional responsibilities may be assigned as needed.
