Part-time Practice Nurse

Job summary The post holder is responsible for the delivery of general practice nursing care to the practice population. Supported by a senior nurse within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities.
They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In addition, the post-holder will be required to support the multidisciplinary team, ensuring the strategic objectives of the practice are met.
Main duties of the job The successful candidate must have the ability to develop, implement and evaluate individual treatment plans for patients with a known long-term condition. Support patients to adopt health promotion strategies that promote patients to live healthily, and encourage principles of self-care, including cervical cytology.
Implement and participate in vaccination and immunisation programmes for both adults and children. In addition to the primary responsibilities, the practice nurse may be requested to: Support the multidisciplinary team as required, Partake in audit as directed by the Clinical Audit Lead and Support the Practice Manager in reviewing and updating clinical standard operating procedures, protocols and policies About us Our practice team is made up of both clinical and administrative staff, who work closely together to deliver care to our practice population.

The clinical team comprises of GP Partners, Salaried GPs, Advanced Clinical Practitioners (Nurse, Paramedic, Pharmacist), Practice Nurse, Healthcare Assistants and Phlebotomist. The practice operates on a team based approach with regular meetings to share best practice and ideas.
Team objectives are clearly stated and measured against national and local criteria.
