The CNS/SP has clinical knowledge in this specialist field and works independently and as part of a multidisciplinary team.

As part of the Respiratory team the CNS/SP will be responsible for the co-ordination and care of patients with respiratory illness to ensure they receive the highest quality care. You will be part of the specialist workforce and use advanced assessment skills and knowledge to review patients in their own home, clinics or in hospital and ensure they receive the best clinical outcomes.
You will ensure that patients discharged from hospital following an admission with their chronic lung condition will have appropriate care and support to avoid readmission, advise and assess patients in the community for home oxygen therapy and respond promptly to community requests for admission prevention for patients suffering an exacerbation of their lung condition.

The job will require working flexible hours (including unsocial hours) and in other designated locations across NE Hampshire, Farnham and Surrey Heath as well as the primary base (Farnham Hospital). In particular, flexibility is required across secondary care and community.
