An opportunity has arisen for a care coordinator (full or part time considered) to work within North Solihull PCN. They will coordinate the work of healthcare professionals and non-clinical staff involved in the care of patients registered at GP practices within our Network.
(Please note this is not a clinical role).

THIS JOB ADVERT IS BEING ADVERSTISED FOR NORTH SOLIHULL PRIMARY CARE NETWORK BY MLCSU

The care coordinator will be part of a team that support 10 independent GP practices working across 14 main and branch sites in North Solihull who have agreed to collaborate to improve services for patients living in this area. With a total population of over 78,000 patients located in various sites across Chelmsley Wood, Smithswood, Kingshurst, Sheldon, Marston Green, and Castle Bromwich we work alongside a range of local providers from primary care, community services, social care and the voluntary sector, to offer more personalised, coordinated health and social care to their local patient populations.

As a PCN, we cover a total population of over 80,000 patients with our purpose being to enable GP practices to work together in a collaborative way to develop and deliver network-based services that respond to the needs of the local population.

Key requirements
 •  Good standard of education GCSE or equivalent in English and Mathematics
 •  Have ICT skills and knowledge of general software applications relevant to the post including use of Microsoft package, and have a good level of competence in use of I.T. in systems such as SystmOne
 •  Good organisational skills and the ability to communicate effectively with a wide range of healthcare professionals and other stakeholders including patients, families and carers.
 •  Enthusiastic and adaptable
