Staff Nurse Urgent Community Response | Sussex Community NHS Foundation Trust

Sussex Community NHS Foundation Trust | Lewes | findajob.dwp.gov.uk |
It’s an exciting time for our Urgent Community Response (UCR) Service as we continue to build on our successful model. UCR services work with patients in their homes across Brighton & Hove and East Sussex communities to avoid hospital admissions and support those being discharged home.

Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. They deliver:

 •  A rapid response to people in the community who are experiencing acute episodes of ill-health.
 •  A supported discharge service to facilitate timely hospital discharges.
 •  Acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

We are looking for Staff Nurses to join us, so if you’re a motivated and enthusiastic nurse looking to be part of a supportive team and use your skills to provide excellent cutting-edge urgent community care, we’d love to hear from you!

Main duties of the job

The focus of this role is delivering high quality care in the community, working with patients towards re-enablement, self-management, and independence throughout the continuum of care. You will do this by delivering holistic assessments, planning, implementing, and reviewing care provision.

You will be part of a team of colleagues supporting evidence-based care, facilitating staff learning and development. You will provide nursing input for patients including medication management, wound care, catheter care and identifying deteriorating patients.
This post is an ideal opportunity for a nurse who is keen to develop both their holistic assessment skills and knowledge of urgent community response.

If you are new to Sussex Community NHS Foundation Trust, you will join our preceptorship programme, through protected time and regular clinical supervision with dedicated registered professional support.

We are the main provider of NHS community services across East and West Sussex. Our 6,000 staff serve a population of 1.3 million providing essential medical, nursing, and therapeutic care to adults and children.

Our Trust vision is to provide excellent care at the heart of the community, helping people plan, manage, adapt to changes in their health, supporting avoidable hospital admissions and reducing hospital stay times.

We have opportunities for everyone across our wide variety of services including medical, clinical, support and corporate services.

Why work for us?
 •  Positive 2023 NHS Staff Survey results, scoring highly in compassionate culture & leadership, and staff wellbeing support
 •  Variety of working environments across the community, in patients' homes, within our community hospitals and bases across the county
 •  Supportive flexible working patterns such as: part time, flexi time, annualised hours, and flexi retirement options
 •  Excellent training and development opportunities
 •  Research opportunities
 •  Cost-effective workplace nurseries in Crawley, Hove and Brighton
 •  Thriving BAME, Disability and Wellbeing, LGBT+ and Religion and Belief staff networks
 •  Level 3 accredited Disability Confident Leader, committed to creating inclusive workplaces
 •  Veteran Aware Trust, achieving accreditation from Veterans Covenant Healthcare Alliance (VCHA)
 •  Located in stunning Sussex, with easy access to the South Downs and the coast
 •  Access to a wide range of benefits and discounts for NHS staff

You will need to able to drive with a full UK license and access to a vehicle due to the geographic area covered by the team.
