The medical practice situated in the vibrant community of Reading is dedicated to delivering top-tier healthcare services with a focus on patient well-being. Committed to excellence and innovation, the practice is currently seeking a skilled Practice Nurse to join its dynamic team.

Role Overview:

The Practice Nurse will assume a pivotal role in administering comprehensive nursing care, ensuring patients receive optimal support for managing their health. Collaborating closely with a multidisciplinary team, the Practice Nurse will uphold the practice's commitment to evidence-based care and health promotion.

Key Responsibilities:

Conducting nursing assessments and delivering clinical interventions according to established guidelines.
Managing chronic conditions such as diabetes, hypertension, and asthma through regular monitoring and patient education.
Administering vaccinations and immunizations in adherence to national schedules.
Performing wound care, dressing changes, and minor surgical procedures as necessary.
Assisting patients in implementing lifestyle modifications to enhance health outcomes.
Maintaining accurate patient records using electronic medical record systems.
Collaborating with GPs, healthcare assistants, and other team members to ensure coordinated patient care.

Requirements:

Registered Nurse with current NMC registration.
Previous experience in a Practice Nurse or similar primary care role is preferred.
Proficiency in nursing assessments, vaccinations, and chronic disease management.
Strong communication skills, capable of engaging patients from diverse backgrounds.
Excellent organizational abilities and adept at managing multiple priorities.
Familiarity with electronic medical record systems is advantageous.
