We have partnered with a cutting-edge healthcare partnership and training practice in South-East London. They have a list size of approx. 21,000 patients and are dedicated to improving patient services and training and developing clinicians for the future.
They are actively seeking a new Clinical Pharmacist to join their diverse and experienced team of healthcare professionals.
Position Details:
• Job Title: Clinical Pharmacist
• Job Type: Full time/part-time hours available from Monday – Friday
• Location: South-East London
• Salary: £45,000 - £55,000 per annum, depending on experience, with an extensive benefits package including learning and development opportunities.
• Additional Services: Complete range of medical services, including maternity care, baby clinics, immunisations, general medical and surgical care, minor surgery, contraception, screening, and health promotion for both men and women.
• Support Team: Work alongside four GP Partners, Salaried GPs, Nurse Practitioners, Practice Nurses, Physician Associates, and Healthcare Assistants.
About You:
• GPhC registered pharmacist with experience working within a GP Practice setting.
• Experience and an awareness of common acute and chronic conditions.
• Effective communication of complex information via various channels.
• Strong interpersonal, organisation, and IT skills, capable of challenging managerial and clinical practices.
• May hold or be working towards an independent prescribing qualification is desirable but not essential as full training provided.
Main Duties:
• Work as part of a multi-disciplinary team to clinically assess and treat patients, utilising expert knowledge of medicines for specific disease areas.
• Manage care for patients with chronic diseases, conducting clinical medication reviews to proactively handle complex polypharmacy cases.
• Ensure accurate recording on clinical systems to meet QoF compliance, while contributing to meeting targets and enhancing services.
• Provide leadership on medicines optimization, quality improvement, and adherence to guidelines, while maintaining a clean and effective working area.
