Discharge Co-ordinator | Sherwood Forest Hospitals NHS Foundation Trust
Are you ready for a challenge? The Discharge Team is seeking to appoint Discharge Coordinators. The role of the Discharge Coordinator will be to support the Discharge to Assess process, facilitating safe discharges from the hospital and aiding flow and capacity within the Trust.
This position entails a seven-day service, including bank holidays. Current shift patterns will be a variety of 8:00 AM to 4:00 PM, 9.00AM to 5.00PM and 10:00 AM to 6:00 PM. The role involves working across all clinical areas and may require duty at other Sherwood Forest sites (Mansfield Community Hospital and Newark Hospital) as needed.
Candidates should possess a clear understanding of the Discharge to Assess process, sound knowledge of discharge pathways, and awareness of social care and health processes. Full training will be provided for this role
