Practice Nurse

MAIN PURPOSE OF JOB: To support care given by senior clinicians in the practice, by working alone and as part of the team, following practice protocols. Providing screening opportunistically and through structured health promotion clinics, combined with support and advise to patients, in order to achieve change in unhealthy behaviour, and contribute to the implementation of the National Service Frameworks and chronic disease management.
To assist in clinical audit and the setting and monitoring of standards of care. Teach and direct others in the practice including unqualified staff. Participate in providing evidence-based practice and work effectively in the team. SPECIFIC OBJECTIVES/AIMS: The post holder will be able to work alone and as part of a team, following practice protocols.
Be aware of current evidence based practice and standards. Demonstrate this in supporting the care and service provided by the senior clinicians. Support the senior practitioners to deliver nursing care to meet the needs of the practice population in accordance with agreed practice protocols.
Through use of evidence-based practice, perform wound care using aseptic technique on uncomplicated wounds. Undertake health screening, and venopuncture. Assist in addressing health promotion and public health strategy, with the maintenance of disease registers and provision of support and advice to patients within health promotion.
Support patients and colleagues in adopting sound infection control measures. To assist in delivering care for specific diseases and health promotion, following completion of accredited training courses, or supervised experience to demonstrating competencies relating to the following, Uncomplicated wound management.
Administer injections under Patient Group Direction or individualised prescription. Assist in the immunisation of children. Be competent in anaphylaxis and resuscitation. Ear syringing following assessment by a senior clinician. Child health and child protection. Mens health. Support the senior clinicians in delivering chronic disease management.
Womens health screening, i.e. cervical smears and family planning. OTHER DUTIES Ensure safe storage, rotation and disposal of vaccines. Ensure the decontamination of instruments and clinical equipment according to local and national guidelines.
To be aware of current evidence based approaches to patient care and demonstrates this through continuous improvement of patient care. The post holder will be expected to maintain strict confidentiality at all times and be aware of Caldicott principles.
The post holder will be expected to familiarise themselves with the health and safety poster of the practice and maintain the responsibilities outlined. The post holder will be expected to remain abreast of current evidence based practice and make use of the learning opportunities offered.
The post holder will be expected to work within the scope of their professional competencies and have relevant post registration qualifications. Infection control is a significant part of the role with cleaning and sterilising of instruments, and management of clinical waste.
