You will receive comprehensive support to develop key competencies, including initial patient assessments, medication reviews, patient education, and collaboration with cardiac exercise specialists to deliver individualised exercise programs.

This role is community-based and requires travel to various locations across Salford, including the acute care site.

Main duties of the job

The successful candidate will be responsible for the specialised assessment and management of patients with cardiovascular disease (CVD).

As an integral member of the multi-disciplinary Cardiovascular Rehabilitation (CVR) team, the post holder will contribute flexibly to the delivery of exceptional care for patients and their families across both primary and secondary care settings.

This role encompasses inpatient ward visits, conducting face-to-face clinical assessments, providing home visits for patients, supporting the implementation of CVR classes, and overseeing the ongoing case management of patients.

Additionally, the post holder will participate in service development initiatives and engage in research activities to enhance the quality of care provided.

Essential criteria
  • Current NMC Registration RGN

Qualifications

Essential criteria
  • Registered General Nurse
  • First Degree in Nursing or Nursing Diploma or an equivalent portfolio of evidence
  • Substantial years post registration experience within acute cardiology/cardiothoracic or cardiovascular rehabilitation
Desirable criteria
  • Post graduate training /courses in specific areas appropriate to the post e.g. teaching qualification
  • Supplementary/Extended nurse Prescribing Qualification
  • Have achieved or be working towards a masters

Knowledge,Training & Experience

Essential criteria
  • Demonstrates specialist knowledge of CVR/CVD current legislation and the management of patients.
  • Evidence of ability to analyze complex situations using judgmental skills to recommend best course of action.
  • Be an effective multidisciplinary team member who is able to relate to colleagues at all levels of the trust with tact and diplomacy.
  • Evidence of involvement and leadership in teaching and mentoring learners .
  • Evidence of audit ability and involvement
  • Evidence of the use of clinical systems and to be computer literate

Skills & Abilities

Essential criteria
  • Good understanding of computers & applications for example word, outlook
  • Excellent verbal and written skills
  • Proven organisational skills including time management, effective delegation and prioritisation and the ability to complete a task
  • Able to work autonomously within a clinical area with senior support
  • Able to present information in a clear and logical manner
  • Ability to travel to different geographical locations across the Trust footprint in line with service needs
Desirable criteria
  • Full UK driving license with willingness to use own car for work purposes
