“I require assistance to access the community effectively. This includes support for outings such as shopping trips and participation in social events, where navigating the environment can be challenging. Due to my diagnosis of Branchio-Oculo-Facial Syndrome, I stand at 3 feet 10 inches tall, which can also influence my interactions with the world around me.

As a result, I find it necessary to have a support worker accompany me when I venture outside.

My dietary needs are fulfilled at night through a PEG (Percutaneous Endoscopic Gastrostomy) feed, ensuring I maintain proper nutrition during sleep. A support worker plays a crucial role in helping me manage various aspects of my home and personal life to ensure I maintain my independence and well-being.

Their responsibilities include assisting me with household tasks such as tidying up the living room, organising and putting away clothes, and ensuring that my home remains a comfortable and welcoming environment.

In addition to helping with housework, the support worker will accompany me to health professional appointments, including visits to my doctor or mental health therapy sessions, as well as other essential appointments that may arise. This support will help me navigate my medical needs more effectively, such as organising my medication and PEG feeding equipment.

Furthermore, they will join me on outings to carry my belongings, fit the booster seat in public transport and cars, monitor obstacles for my mobility scooter, and put it away. Additionally, they can help me engage in social activities and community events, which are vital for my mental and emotional well-being.

I actively participate in all Deaf events, and having the right support is crucial to fully engage in these activities. The support worker will assist me with managing correspondence, including writing letters and handling general emails, to ensure I stay informed and connected with the world around me.

They will also help me note, retain, and manage the volume of information I process daily due to my health and social needs, assisting my decision-making through information recall. This will help me maintain the emotional and mental support I also require.

The support worker must be proficient in British Sign Language (BSL), as this is my primary mode of communication. Communicating effectively in BSL will enable me to feel included in conversations and social interactions, enhancing my overall quality of life.

This support will significantly contribute to my ability to maintain independence while participating fully in my community.”
