The Post provides embedded physician support to elective and non-elective general, colorectal and urological surgery on the Charing Cross site in addition to providing rotational coverage to Albert Ward (surgical step down ward) on the St Mary’s site.
The ICHT Surgical Liaison Service consists of a team of consultant geriatricians who provide embedded medical care to adults of all ages admitted under surgical specialties;major trauma, orthopaedics, vascular surgery, general surgery, neurosurgery and spinal surgery.By effectively addressing medical needs and complexity alongside acute surgical issues they help to provide comprehensive holistic care, prevent complications, optimise outcomes, and give every patient the best chance of making a successful recovery.

This clinic helps evaluate and prepare patients for surgery, providing them, their relatives, and the surgical and anaesthetic teams with all the necessary information to make informed decisions about surgery, perioperative care, and post-operative recovery.
Key features of the Surgical Liaison Service include:
- Preoperative assessment and optimisation of frailty, cognitive disorders, and multimorbidity.
- Prognostication and shared decision-making.
- Management of postoperative complications, hospital-acquired deconditioning, and cognitive disorders.
- Rehabilitation, goal setting, discharge planning, and referral to community services.
- Treatment escalation and advance care planning.
- Clear communication with patients and carers throughout the perioperative pathway.
- Collaborative working with other disciplines and specialties across the Trust.
