Discharge Planning & Transport Co-ordinator - Lister Hospital
A Vacancy at East of England Ambulance Service Trust.
To provide proactive and responsive practical and administrative support to ensure the efficient booking of transport for non-emergency patients.
To supervise the running of day to day Patient Transport and be EEAST's on site lead managing and prioritising demand during busy periods, whilst complying with the local Acute Trusts expectations.
To focus on operationally supporting the Hospital to expedite patients home through patient transport.
The post holder will be expected to efficiently liaise with internal teams at the hospital and the Non-Emergency Control room.
The role is to provide proactive and responsive practical and administrative support to ensure the efficient booking of transport for non-emergency patients travelling to/from healthcare settings.
To supervise the running of day to day discharges and be EEAST's on site lead managing and prioritising demand during busy periods, whilst complying with Hospital Site expectations.
To focus on operationally supporting the Hospital to expedite patients home through patient transport.
The post holder will be expected to efficiently liaise with internal teams at Hospital and the Non-Emergency Control room.
Alert appropriate colleagues within both the hospital and EEAST organisation to anticipated difficulties relating to transport for patients being discharged or transferred and assist in resolution of the same.
You will work along side the hospital and control using your knowledge and assessments to get the best results for discharges and transfers
The post holder will be an integrated member of the transport team, maintaining the crucial link between ED, Acute Care Units, wards, clinics and the site team.
