Community Rotational Physiotherapist

North Manchester Community Services have a vacancy on their Community Rehabilitation Rotation and are looking to recruit an enthusiastic and dynamic physiotherapist. The successful candidate will have a passion for delivering care close to home and be committed to keeping patient's in their own locality.
Physiotherapists will rotate through a diverse range of areas:
Community Rehabilitation Team: delivered over 5 days, Monday to Friday.
Manchester Community Response (MCR) – North (Home Pathway, Discharge to assess and Crisis response): delivered over 7 days, Monday to Sunday 8:00-20:00.
Community Stroke Rehabilitation: delivered over 5 days Monday to Friday 08:00-16:30
Crumpsall Vale Intermediate Care - delivered over 7 days, Monday to Sunday.

UK driving license and access to own vehicle are essential for the post.
Main duties of the job
The post holder will be responsible for providing specialist complex multifactorial clinical assessments, treatment and advice to patients in the community who have a variety of general presentations such as orthopaedic, musculoskeletal, neurology, respiratory, falls and amputees as part of a community rehabilitation rotation.
The individual will manage a clinical caseload of clients, including those with complex and highly complex needs with support from the band 7 therapists for highly complex patients and supervise the band 3 and 4 support staff with caseload management.
The post holder will work without direct supervision as a lone worker in the community assessing, planning, implementing, evaluating and modifying individualised treatment programmes in a variety of care settings e.g. patients home, clinic setting, residential homes and intermediate care setting for patients with low to highly complex needs.
