PLEASE REFER TO THE ATTACHED JOB DESCRIPTION FOR FULL DUTIES. To work within the Integrated Care Team, providing care to patients in their own homes / care home / other care settings providing assessment and treatment of patients in the community over a 8am-8pm period 7 days a week.
It will also support appropriate early discharge of patients and will give support to Primary Care and the Unplanned Care Team. To carry out planned and unplanned health (nursing) interventions as designated by the lead clinician, and will work both supervised and unsupervised in patients own homes, residential, nursing and other intermediate care facilities, reporting back patient progress and informing the lead clinician of any problems.
