Job overview

To work as a member of the 24/7 Community Nursing Team - assessing, planning, implementing and evaluating care under the direction

of the Team manager/ Leader.

Providing nursing care to patients, of the Blaenau Gwent area, in their own homes, working collaboratively with the MDT to support a seamless high quality care provision to the patients we support in the community. Supporting the strategic direction and models of service delivery working across PCCS.
Deputise for the District Nursing Team Leader/Deputy in their absence

To act in accordance with the NMC Professional Code of Conduct and other policies

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

Clinical Skills:

To support patients clinical needs within the community a diverse clinical skill set is required.

These include: diabetic management, palliative and end of life care, complex wound care, care/management of a variety of surgical drains, pressure area care/management, bowel care/management, catheter care/management, dietary care/management, health promotion, equipment provision/review, venepuncture, IV therapy, enteral feeding and gastrostomy tube care/management, tracheostomy care/management, ability to undertake verification of expected death, (or willing to work towards).

To work as a member of the 24/7 Community Nursing Team - assessing, planning, implementing and evaluating care under the direction
of the Team manager/ Leader.
  • This post will ensure the provision of effective, efficient and appropriate high quality patient care
  • Supporting the strategic direction and models of service delivery working across PCCS.
  • Deputise for the District Nursing Team Leader/Deputy in their absence
  • To act in accordance with the NMC Professional Code of Conduct and other policies Communication
  • To act as an advocate for patients and significant others.
  • To communicate sensitive with patients utilising empathy, persuasion and reassurance when there may be barriers to understanding
and for those with special needs.
  • To liaise and communicate with members of the multidisciplinary team in the planning and delivery of patient care from admission to the
caseload through to discharge, under the direction of the Team Leader.
Patient Client Care
  • To assess, plan, implement and evaluate care under the direction of the Team Leader. Making judgement on patients’ conditions and in
relation to further investigation and to develop programmes of care.
  • Competence in a range of clinical skills, e.g. IV additives, suprapubic catheterisation, cannulation, venepuncture, VOED etc.
Clinical responsibilities
  • To accept professional responsibility for both first visit and delegated care.
  • To be committed to and take responsibility for the quality of care delivered to the patient caseload.
  • To participate in the formal monitoring of standards.
  • To maintain accurate records/standards for documentation including IT.
  • To participate in clinical audit and data collection ensuring report and information are delivered on time.
  • Contribute to the development and delivery of the corporate nursing agenda.
  • To undertake any other duties consistent with the overall grade/responsibilities of the post.
Improvement and Monitoring
  • To act as a mentor/preceptor to students and junior staff.
  • To contribute to meeting the learning needs of team members, sharing expertise and evaluating training undertaken.
  • To demonstrate a continuing process of attaining educational, research, clinical and leadership skills.
  • To improve current practices promoting research-based practice.
Planning & Organisational Skills
  • To plan the provision of care for patients.
  • To ensure that services are provided in a safe and sustainable way by appropriately trained and supported clinicians..
  • Be aware of and adhere to Health Board procedures:
  • Health and Safety at Work Act; Data Protection Act 1998;
  • Other relevant legislation and agreed practice and policy.
Management, and/or Training
  • To provide leadership and supervision of junior staff, support workers and students.
  • To deputise in the absence of the Team Leader/Deputy.
  • To be aware of budgetary implications when prescribing care.
  • To contribute to the annual Personal Reviews of junior colleagues, identifying training needs and developing Personal Development Plans.
  • To be familiar and comply with the Health Board’s Policies and Procedures.
  • To successfully manage the introduction of chances in practice within the team.
  • To attend and participate in team meetings.
  • To be aware of and participate in the delivery of the Divisional Clinical Governance agenda.
  • Contribute to the recruitment and selection process

Qualifications

Essential criteria
  • Registered Nurse (with current NMC registration). Evidence of continuing Professional development. Evidence of Portfolio Development
Desirable criteria
  • Diploma Level Education. District Nursing Qualification

Desirable criteria
