Newham Diagnostic Memory Clinic
We are looking for an experienced and enthusiastic Mental Health professional with leadership and supervisory skills to join our Diagnostic Memory Clinic. The post holder should have specialist interest in Dementia and background experience of working with older adults in community mental health setting.
We are currently undergoing an exciting time of transformation, working in partnership with our service users, their carers, Local Authority and voluntary organisations in a drive to continue our goal of providing inclusive and high quality care while developing our service beyond the traditional scope of Diagnosing.
To support us in this process, the successful candidate will be working alongside the Operational Team Lead to provide day-to-day management and clinical leadership to the Team, supporting staff to adapt to the changes and making complex decisions regarding the planning, development and monitoring the care provided.
Main duties of the job
The holder will work alongside the Operational Team Lead as senior practitioner to manage the Diagnostic Memory Clinic within the Community Mental Health Care for Older People of the Newham Locality
Demonstrate ability to manage a service and group of staff, ensuring that standards and quality of care provided are (culturally) sensitive to individual service users needs and have positive impact on their experience and that of their carers’.
Ensure holistic assessment, care and support to people referred to the service for memory assessment, those presenting with co-morbid health care needs.
Effectively coordinate and manage caseload of service users referred and /or waiting for memory assessment, ensuring safety and responsiveness of the Service.
Ensure that service users and their carers are well informed and actively engaged in all stages of the assessment and care delivery process provided by the Service, ensuring that their views are acknowledged and taken into account.
Ensure post-diagnostic feedback and Support, including therapeutic counselling and groups are offered to service users diagnosed with dementia and/or cognitive impairment and their families, ensuring that the support provided are appropriate to meet individual needs.
To manage Nurse-led Titration Clinic run by the Service.

Essential criteria- • Registered Mental Health Nurse with current Registration with NMC
- • Evidence of Post Registration continuing professional development
- • Advanced level specialist Dementia nursing skills
Experience
Essential criteria- • Previous specialist nursing experience at Band 5 and above.
- • Previous specialist nursing experience of working with older adults
- • Previous experience of functioning as a team member
- • Previous experience of independently conducting psycho-social assessment and planning care for service user with complex care needs
- • Previous experience of acting upon service user and carer feedback and information to improve service delivery, taking personal responsibility for resolving the situation
- • Previous experience of willingness to take responsibility and accountability for decisions and actions as a leader.

Essential criteria- • Understanding of the Mental Capacity Act (2005) and how the Act is used to support older adults with Dementia and cognitive impairment
- • Understanding of the Mental Health Act 1983 (2007) and how sections of the Act are used in practice
- • Understanding of Human Rights Act 1998 and how this is used in practice when caring for people with dementia and cognitive impairment
- • Knowledge of Trauma-informed Care
- • Knowledge and understanding of safeguarding vulnerable adults and its procedures
- • Understanding of the key themes of clinical governance and evidence-based practice.
