We are currently seeking a dynamic Social Care Coordinator to join our Mental Health Social Care Team.

In the role of Social Care Coordinator at Reading Borough Council, you will play a crucial role within a friendly, experienced and forward-thinking team. You will collaborate with other Social Care Coordinators, Social Workers, Occupational Therapists and professionals from the voluntary and community sector.

We work with people with forensic histories, carers and families, people with complex mental health difficulties and we have a re-enablement service within our team. We also currently have a Hoarding Project. We seek creative ways to work together to meet the needs of people who use our services together.

The Mental Health Social Care Team aims to actively promote wellbeing and independence, working with people with complex mental health difficulties in partnership with health colleagues. Supporting people to retain or regain their skills and confidence by using existing and new support networks, Technology Enabled Care (TEC), equipment and short-term interventions.

About the Role:

This is a core role withing the team to assist with the health and wellbeing of people who have complex mental health needs. In the role of a Social Care Coordinator, your will work closely with people using a collaborative and strengths-based approach to implement interventions to reduce risks and support positive outcomes.

You will hold a caseload of people with mental health difficulties, and will be expected to work independently with support and supervision from an experienced Social Worker. Due the nature of the work you will need to have a good understanding of risk management; managing yourself in difficult environments; flexible and able to analyse varied information or situations to produce solutions.

You will need to be able to ask for support when needed from colleagues and we work closely as a group to support each other.

You will work within the legal framework provided by the statute, guidance, policies, and procedures.

Role requirements:

Experience working with people with mental health difficulties and or complex needs.

Experience working within the Health, Adult Social Care or voluntary sector – (not essential)
