Care Coordinator - Swindon

Location: SwindonSalary: £29,270 OTE
Hours: 40hrs per week + out of hours shared on call service Office hours Monday-Friday 08:30am -17:00pm. Flexibility in hours may be required to meet the need of the business
Duties can include:
Allocation of care workers to customer visits using effective rostering, ensuring continuity of care and considering locations, skills required.Communicating regularly with care workers, providing guidance and resolving queries in a timely manner
Liaison with other healthcare professionals
Ensure all paperwork is completed and entered onto relevant systems
Maintain integrity when dealing with confidential matters
Approach clients with understanding and empathy, remembering that each client is to be treated as an individual
Essential for role:
Experience of working in health and social care within a community-based setting would be preferableBe computer literate and have good office skills
Be Honest, Caring, Non-judgemental and to act with Kindness and Respect
Be organised and work calmly under high pressure
Desirable:
Health and social care qualifications NVQ 2/3 or aboveValid driving licence with access to a car
