An exciting opportunity has arisen to join our team of Advanced Clinical Practitioners (ACP) in Out of Hours (OOH) Medicine. We are looking to recruit a Trainee Advanced Clinical Practitioner to join our established team of ACP's. You will need to have high level of experience in acute health care and have sound clinical knowledge whilst also demonstrating managerial and leadership abilities.

Trainee practitioners will be expected to commence an MSc in Advanced Clinical Practice (Advanced Nurse/clinical Practitioner) under the support and guidance of the lead Advanced Clinical Practitioner for Medicine and our medical colleagues.

The tACP’s/ACP’s are pivotal in the triage, assessment and treatment of acutely unwell medical patients out of hours. The OOH ACP is a key leadership role within the OOH medicine team, working alongside both medical and ward based teams to ensure that patients are seen by the right person, at the right time.
Providing clinical expertise across a footprint of over 350 beds, covering all speciality medicine and medicine for older people (MOP). This is an incredibly varied and exciting role. The OOH trainee Advanced Clinical Practitioners (tACP) / Advanced Clinical Practitioners (ACP) are central to the OOH multidisciplinary team.

This role involves undertaking capability based patient assessment and examination in order to formulate a management plan for patients admitted to UHS for diagnosis and / or treatments.

We are looking for an enthusiastic and experienced nurse or Allied Health Professional with a passion for Medicine. Experience of independent clinical assessment, practice and decision making is advantageous.

If successful, you will play a key part in delivering specialist Out of hours training and education to ward based multi-professional teams across the Trust. Practitioner’s are supported to undertake quality improvement / audit projects to further improve patient care and service delivery.

The successful candidate will provide clinical leadership and be an excellent role model for the multi-professional teams they are working in collaboration with.

The OOH service currently covers twilight and night shifts across Monday-Friday nights. And 24/7 provision at the weekends and on bank holidays, through a shift pattern of long days, twilights and nights.
