Job overview
The Complex Care Team proactively manage a caseload of identified patients, identifying what matters most to them and developing personalised care plans.
The team will consist of Advanced Care Practitioners from Sirona, GPs, social prescribers and voluntary sector representatives. They will make sure that appropriate support is made available to the patient and their carers. They will be the first point of call for this identified cohort, proactively managing their needs, and coordinating the Multidisciplinary Team to meet them by facilitating communication between the patient and other professionals across health and social care providers.They will provide holistic and person-centred care and support particularly focussing on the care of older people with long term conditions and those living with Frailty in their own homes.
This team will assist our advanced care practitioners and practice staff in the provision of treatment, preventative care, health promotion and patient education to this cohort of patients co-ordinating the patients journey through the health and social care system.
Main duties of the job- The Care-co-ordinator will function at a higher competency level than that of other health care support workers and work unsupervised, undertaking a range of activities and extended roles having received training and been assessed as competent.
- Undertake holistic assessments for service users as delegated by senior members of the team
- Promote optimum independence through linking the service user and carer to appropriate support
- To provide a safe, patient centred, effective and evidence-based care
- To deliver a full range of clinical, health and social care activities, supported by a robust competency framework to service users within their own home
- Be able to and willing to work flexibly and to travel throughout the service delivery geographical area from a base at Marina Healthcare Centre in Portishead next to our Advanced care practitioner

Essential criteria- Successful completion of relevant Level 5 Foundation Degree.
- Undertake any training relevant to the role.
Skills, Knowledge & Experience
Essential criteria- Experience of direct client care in a health or social care setting.
- Previous experience of working in a community based setting.
- Previous experience using a variety of IT platforms i.e EMIS, email.
