An exciting opportunity has arisen for a clinical pharmacist to join our growing pharmacy team who support our Hospital at Home virtual ward service based at the East and North Herts Referral Hub. The Hospital at Home hub is a dynamic and innovative environment adapting to the needs of the local population and the pharmacy team are integral to the success of the service.
Main duties of the job
The Hospital at Home service (otherwise known as Virtual Ward) can help to prevent unnecessary acute hospital attendances and admissions for a range of conditions by providing medical care and support for patients in their own homes or usual place of residence.
The Hospital at Home service provides same or next day assessment, followed by treatment and monitoring as appropriate. The key services and conditions covered are:- IV/oral antibiotics to treat acute infections
- Heart failure management, including IV diuretics
- COPD and other respiratory conditions
- DVT pathway
- End of life and specialist palliative care
- Support for care homes
- Falls
- IV fluids
- Medication reviews by pharmacists
The service is open to patients with a GP in East and North Hertfordshire who are aged 18 and over, both with and without COVID-19.
The clinical pharmacists work alongside clinicians, Advanced Clinical Practitioners (ACPs), nurses and therapists to support the safe delivery of medical care in the patient’s home. Progression in this role may involve prescribing once suitably trained, so interest in undertaking the qualification to be anIndependent Prescriber would be beneficial and you would be supported to do so in this role if needed.

We are looking for additional pharmacists to support further expansion of the service.
The SystmOne EPMA/EPS system has now been fully implemented across all hospital units, so experience of using this system would be advantageous.
The successful applicant will need to be self-motivated and able to work on their own, whilst also integrating into a well-established team. You will have excellent organisational, interpersonal and communication skills together with a good working knowledge of hospital pharmacy.
The successful candidate must have experience of working in a clinical setting with a multidisciplinary team. Experience of implementing Medicines Optimisation projects will be ideal. You will need to havepostregistration experiencein hospital pharmacyand a clinical diploma orworking towards one.You will also need a great deal of self-motivation, sound clinical knowledge and computer literacy. The role will also be supported by a full training package, according to needs.
